- by Tanis J FermanINTRODUCTION: Visual perceptual difficulties occur in dementia with Lewy bodies (DLB). Normative data and cut scores for DLB and mild cognitive impairment with Lewy bodies (MCI-LB) are needed for the 20-item Noise Pareidolia Task.
- by Karol KutyłowskiINTRODUCTION: Alpha-synucleinopathies are a group of disorders characterized by the accumulation of the pathological form of alpha-synuclein. This group comprises Parkinson's disease, Parkinson's disease dementia, multiple system atrophy, dementia with Lewy bodies, pure autonomic failure, and idiopathic REM sleep behavior disorder. Despite sharing a similar molecular basis, these disorders differ in their clinical presentation. Characteristic […]
- by Lieza G ExaltoIn the future, treatment of dementia will be personalized, targeted to contributing pathologies, rather than the clinical syndrome. Anti-amyloid drugs for Alzheimer's disease are the first approved drugs; however, the impact of co‑pathology on the treatment effects remains incomplete. Currently, only patients with a clinical diagnosis of Alzheimer's disease are eligible for these new drugs, […]
- by Carlo Alberto ArtusiDisease-modifying trials in Parkinson's disease (PD), multiple system atrophy (MSA), and dementia with Lewy bodies (DLB) increasingly incorporate fluid and imaging biomarkers to demonstrate target engagement, de-risk subsequent phases of drug development and provide mechanistic evidence of biological activity. Whether biomarker modulation predicts clinical benefit remains unclear. We conducted a PRISMA-compliant review of phase II […]

- by Byoung Seok YeDopamine transporter (DAT) imaging and α-synuclein seed amplification assay (SAA) are biomarkers for Lewy body disease (LBD). We aimed to identify clinical and biomarker factors associated with SAA positivity in patients with cognitive impairment and frequent motor parkinsonism. Dual-phase DAT positron emission tomography (PET) scans, cerebrospinal fluid (CSF) phosphorylated-tau/β-amyloid ratio (PTAR) measurement, and SAA were […]
- by John-Paul TaylorBackground: There is high unmet need in dementia with Lewy bodies (DLB). Neflamapimod, showed clinical activity against cognitive and motor endpoints at a dose of 40mg three-times-daily (TID) in a prior phase 2a study in DLB, most prominently in those without Alzheimer disease (AD) co-pathology, assessed by plasma ptau181. Methods: 16-week randomized, placebo-controlled clinical trial, […]
- by Rahil N VaknalliCONCLUSIONS: Our findings indicate that HFOs likely arise from different neuronal populations and are generated through distinct mechanisms in these two models. Importantly, this work shows that HFOs are an electroencephalographic abnormality in a model of DLB and supports their potential as a new biomarker for neurodegenerative disorders.
- by Deirdre M O'sheaINTRODUCTION: Dementia with Lewy bodies (DLB) is characterized by progressive cognitive, neuropsychiatric, motor, autonomic, and sleep impairment. Pharmacological interventions developed with disease-modifying intent have entered clinical testing, but their biological rationale and adequacy of evaluation remain uncertain.
- by Marianna RizzoINTRODUCTION: Mounting evidence suggests overlapping pathology across Alzheimer's disease (AD), dementia with Lewy bodies (DLB), and Parkinson's disease (PD), yet its extent and clinical implications remain unclear.
- by Rebecca MillerINTRODUCTION: People with Parkinson's disease (PD) frequently develop dementia. We investigated the relationship between cognitive impairment and the loss of cholinergic axon projections from the nucleus basalis of Meynert to neocortical regions in PD.
- by Tzukit Talα-Synuclein (αSyn) aggregation is a defining pathological feature of synucleinopathies, including Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy (MSA). However, existing in vitro methods to monitor αSyn aggregation often rely on endpoint assays that cannot distinguish monomeric from oligomeric species or capture early aggregation dynamics. Here, we implement an αSyn bimolecular fluorescence […]
- by Emily K EichenlaubCONCLUSIONS: These results have potential to inform the feasibility of future studies investigating balance in individuals with DLB and designing interventions to improve balance control in these individuals, thereby reducing their risk of falling.
- by Chih-Chung HuangCONCLUSION: These findings suggest memantine as a potential therapeutic alternative for DLB patients with autonomic fragility.
- by Andreas Myhre BaunDespite isolated REM sleep behaviour disorder (iRBD) reliably identifying individuals at high risk of developing Parkinson's disease (PD) or dementia with Lewy bodies (DLB), with more than 90% converting, the neurobiological markers of the conversion phenotype are not fully understood. Most previous studies have found univariate imaging biomarkers of phenoconversion, and most of these lack […]
